Spaying and neutering are among the most common surgeries performed on companion animals — and, for decades, the standard advice was simple: do it early, do it for every pet, and do not look back. That guidance has not disappeared, but the conversation has become more nuanced. New research, especially out of UC Davis, has shown that the optimal age for sterilization depends on the species, the sex, and — for dogs — the breed. The benefits are real and well-documented. So are some of the risks. The right decision for your pet is the one made with both in mind, and in partnership with a veterinarian who knows your animal.

What Spaying and Neutering Actually Are

Spaying refers to the removal of the ovaries — and usually the uterus — in a female animal. Neutering most often refers to castration, the removal of the testicles, in a male. Both procedures are performed under general anesthesia and, in healthy young animals, are considered routine. Recovery typically takes 10 to 14 days, with most pets back to normal activity within two weeks.

Both surgeries permanently sterilize the animal and eliminate the hormonal cycles tied to reproduction. That hormonal change is the source of every benefit and every risk associated with the procedures, which is why timing matters.

The Confirmed Health Benefits

The American Veterinary Medical Association (AVMA) and the ASPCA both report that spayed and neutered pets, on average, live longer than intact ones, in large part because of disease prevention.

For females. Spaying eliminates the risk of pyometra — a life-threatening uterine infection that affects roughly one in four intact female dogs by age 10 — and prevents ovarian and uterine cancers. It also dramatically reduces the risk of mammary tumors, which are malignant in about half of dogs and roughly 85 percent of cats. Epidemiologic studies in cats have found that spaying before the first heat cycle reduces mammary cancer risk by 91 percent compared to intact females, with the protective effect declining the longer the cat remains intact.

For males. Neutering removes the risk of testicular cancer entirely and lowers the risk of certain prostate problems, including benign prostatic hyperplasia (an enlarged prostate that becomes very common in older intact dogs). It also reduces the incidence of perianal tumors and perineal hernias.

Spaying and neutering also produce population-level benefits that no individual surgery can match. Millions of unwanted dogs and cats enter shelters every year. Reducing accidental litters is, by a wide margin, the single most effective way pet owners can help.

The Behavioral Benefits

Behavior change after sterilization is not absolute, but it is real and often substantial. Neutered males tend to roam less, urine-mark less, and mount less. Intact male cats are notorious for spraying strong-smelling urine to mark territory — neutering before sexual maturity prevents this habit from forming in most cats and reduces it in many adults who already spray.

Spayed females no longer cycle through heat, which in dogs means no more bloody discharge every six months and, in cats, no more loud, persistent yowling that can last for days. Eliminating heat cycles also removes the risk of escape attempts and unplanned pregnancies, both of which spike when intact females are in season.

What sterilization is not, despite a persistent myth, is a fix for behavioral problems unrelated to reproductive hormones. Neutering will not reliably cure leash reactivity, resource guarding, separation anxiety, or fear-based aggression. Owners hoping for a personality reset are often disappointed.

The Risks Worth Knowing About

The downsides of sterilization are mostly metabolic and orthopedic, and they show up across both species.

Weight gain. Sterilization lowers resting metabolic rate and increases appetite. Food intake can rise by roughly 20 percent within the first week after surgery. Without portion adjustments, weight gain follows, and obesity is now the most common nutritional disorder in companion animals. Some studies have found neutered dogs are 1.6 to 3 times more likely to become obese than intact ones, and the same pattern holds in cats.

Urinary incontinence in spayed female dogs. Estimates vary, but published studies report that roughly 4 to 20 percent of spayed female dogs develop urethral sphincter mechanism incompetence, which causes leaking, often years after surgery. Large breeds — Boxers, Dobermans, Rottweilers, Old English Sheepdogs — are at higher risk than small breeds. The condition is usually manageable with medication.

Joint disorders and certain cancers in some dog breeds. This is the area where recommendations have shifted most. A series of long-term studies from UC Davis, led by Drs. Benjamin and Lynette Hart, has found that in some breeds — particularly large and giant breeds — neutering before about 12 months of age is associated with a 2 to 4 times higher risk of joint disorders such as hip dysplasia, cranial cruciate ligament rupture, and elbow dysplasia. The Hart team's 2020 study examining 35 breeds found that small breeds generally showed no increased joint risk from early neutering, while many large breeds did. The effect appears to relate to growth-plate closure: removing sex hormones before bones finish developing can leave joints more vulnerable.

The same UC Davis research has linked early neutering to higher rates of certain cancers in some breeds, including lymphoma, mast cell tumors, hemangiosarcoma, and osteosarcoma. Findings are breed-specific, and the picture is still evolving as more breeds are studied.

The Cat: A Simpler Story

For cats, the consensus is clear and largely uncomplicated. The AVMA, the American Association of Feline Practitioners, the Association of Shelter Veterinarians, and the Winn Feline Foundation jointly endorse the "Fix by Five" recommendation: spay or neuter cats by five months of age. The ASPCA notes that the surgery is considered safe in kittens as young as eight weeks when performed by an experienced veterinarian, a practice commonly used in shelters.

The reasoning is practical. Cats can become sexually mature as early as four months of age, and unsterilized kittens can produce kittens of their own before their first birthday. Sterilization before five months prevents the development of urine spraying in most males, almost entirely prevents mammary cancer in females, and eliminates the risk of pyometra. Long-term studies of pediatric neutering in cats have not found the joint or cancer trade-offs seen in large-breed dogs, which makes the case for early surgery straightforward in this species.

The Dog: A Conversation With Your Vet

For dogs, the question is no longer just "spay or not?" but "spay or neuter when?" Current evidence supports a tiered approach based on size and breed.

Small-breed dogs (under about 20 kg expected adult weight). Most evidence supports sterilization around six months of age. Joint disorder risk does not appear to increase, and the benefits — reduced mammary cancer risk, no pyometra, no testicular cancer — are well established. A few small breeds, including the Boston Terrier and Shih Tzu, may have slightly elevated cancer risks with early neutering, which is worth discussing with your vet.

Large and giant-breed dogs. The Hart UC Davis guidelines, now widely cited in veterinary practice, recommend waiting until at least 12 months — and in some breeds 18 to 24 months — before neutering males, and considering similar delays for females. Breeds in this category include the Golden Retriever, Labrador Retriever, German Shepherd, Rottweiler, Bernese Mountain Dog, and Great Dane, among many others. The trade-off is real: delaying spay does mean a slightly higher cumulative risk of mammary tumors, so individual decisions should balance that against breed-specific joint and cancer risks.

Mixed-breed dogs. Use estimated adult weight as the guiding factor. A 25-kg mixed-breed dog should be treated more like a large-breed candidate than a small one.

Alternatives to traditional spay and neuter — including ovary-sparing spay (which removes the uterus but leaves the ovaries) and vasectomy in males — preserve sex hormones while eliminating reproduction. These options remain less common but are increasingly available and may be worth discussing for some dogs.

Surgery, Recovery, and What to Expect

Pre-surgical bloodwork screens for hidden health issues. The procedure itself takes 20 to 90 minutes depending on species, sex, and complexity, and most pets go home the same day. Expect an Elizabethan collar, restricted activity, and pain medication for about two weeks. Watch the incision daily for redness, swelling, or discharge, and call your vet if anything looks off.

Plan to reduce daily calories by roughly 20 percent in the weeks after surgery, and weigh your pet monthly for the first year. The metabolic shift is real, but it is manageable with portion control and consistent exercise — both of which protect against many of the conditions sterilization has been linked to.
