Hypothyroidism is one of the most common endocrine diseases in dogs — and, paradoxically, one of the most over-diagnosed. The classic patient is a middle-aged Golden Retriever or Labrador who has slowed down, gained weight despite a steady diet, and started losing patches of dull, dry coat along the flanks. The story is so familiar that vets are sometimes tempted to treat on a hunch, but a single low thyroid value is not enough — and the wrong call commits a dog to a lifelong medication they may not need. This guide explains what hypothyroidism actually is, who is at greatest risk, the warning signs every owner should recognize, and the evidence-based way it is diagnosed and treated.
What Hypothyroidism Actually Is
The thyroid is a small, two-lobed gland in the neck that produces the hormones T4 (thyroxine) and T3 (triiodothyronine). Together, these hormones set the metabolic rate of nearly every cell in the body. They influence how fast the heart beats, how warm the body stays, how the skin and coat regenerate, how cholesterol is processed, and how energetically a dog moves through the world. When the gland fails, all of these systems slow down at once.
More than 95% of canine cases are primary hypothyroidism, meaning the thyroid gland itself is damaged. The two main mechanisms are lymphocytic thyroiditis — an immune-mediated process in which the body's own white blood cells gradually destroy the gland — and idiopathic atrophy, in which thyroid tissue is replaced by fat for reasons not fully understood. By the time a dog shows clinical signs, roughly 75% of the gland has typically been lost. Secondary hypothyroidism, caused by pituitary disease, and congenital hypothyroidism in puppies are both rare.
Which Dogs Are at Highest Risk
Hypothyroidism most often shows up in dogs between four and ten years old. It is overwhelmingly a disease of medium and large breeds, and several lines carry a clear genetic predisposition. The Merck Veterinary Manual and Cornell University's Riney Canine Health Center list the Golden Retriever, Doberman Pinscher, Irish Setter, Cocker Spaniel, Miniature Schnauzer, Dachshund, Boxer, and Airedale Terrier among the most over-represented breeds. A large screening study published in the Journal of the American Veterinary Medical Association found that the breeds with the highest prevalence of thyroid autoantibodies included the English Setter, English Pointer, Old English Sheepdog, Boxer, and Maltese.
One important wrinkle: sighthounds — Greyhounds in particular, but also Salukis, Whippets, Basenjis, and Scottish Deerhounds — have naturally lower baseline thyroid hormone concentrations than other breeds. A "low" T4 in a healthy Greyhound may be perfectly normal. Vets familiar with sighthounds know to interpret thyroid panels against breed-specific reference ranges, which is why a second opinion is worth seeking if a low-T4 result is the only evidence pointing to disease.
Warning Signs Owners Should Watch For
Because thyroid hormone touches almost every body system, the signs of hypothyroidism are diverse — but most owners describe a similar picture. The dog who used to greet the day with enthusiasm now sleeps through breakfast. The food bowl looks the same, but the dog gets rounder. The coat starts to look dull and the skin starts to flake. The most common changes include:
- Lethargy and reduced exercise tolerance — a dog that lags on walks, wants to come home early, and seems "older than his years."
- Weight gain without an increase in food intake — often the first thing owners notice.
- Cold intolerance — seeking out warm spots, shivering at temperatures that didn't bother the dog before.
- Symmetrical hair loss on the flanks, tail ("rat tail"), and the bridge of the nose, with the head and legs usually spared.
- Dry, dull, or greasy coat that takes a long time to regrow after clipping.
- Hyperpigmentation — patches of darker skin, especially in areas of friction.
- Recurrent skin and ear infections, often bacterial or yeast, that respond poorly to repeated treatment.
- Slow heart rate (bradycardia) on physical exam.
- Neurological changes in advanced cases — a sad-looking "tragic facial expression" caused by myxedema, weakness, or even peripheral nerve problems such as facial nerve paralysis or vestibular signs.
No single sign confirms the diagnosis. The most useful pattern is the combination of weight gain, lethargy, and a deteriorating coat in a middle-aged medium or large-breed dog whose appetite has not changed. Bloodwork in these dogs frequently shows mildly elevated cholesterol and a non-regenerative anemia — both classic but non-specific clues.
Why Diagnosis Takes More Than One Test
Hypothyroidism is famously easy to over-diagnose because almost any serious illness will lower a dog's T4. This is called euthyroid sick syndrome or "non-thyroidal illness," and it can occur with infection, kidney disease, Cushing's, diabetes, cancer, or even heavy stress. Several common medications — including phenobarbital, sulfonamide antibiotics, and corticosteroids — also suppress thyroid hormone levels. Treating these dogs with thyroid hormone does not address the real problem and may delay the correct diagnosis.
For this reason, the 2023 AAHA Selected Endocrinopathies of Dogs and Cats Guidelines emphasize that hypothyroidism should not be diagnosed from a single blood value, and especially not from a low total T4 alone. The recommended workup typically includes:
- Total T4 (TT4) — a sensitive but non-specific screen. A normal TT4 makes hypothyroidism unlikely; a low TT4 means more testing is needed.
- Free T4 (fT4) by equilibrium dialysis — less affected by non-thyroidal illness and a better reflection of true thyroid function.
- Canine TSH (cTSH) — usually elevated in primary hypothyroidism as the pituitary tries to push the failing gland harder. Importantly, cTSH is elevated in only about 63–85% of confirmed cases, so a normal TSH does not rule the disease out.
- Thyroglobulin autoantibodies (TgAA) — supportive evidence of underlying lymphocytic thyroiditis, particularly useful in at-risk breeds.
The most confident diagnosis combines compatible clinical signs, a low fT4, and a high cTSH, ideally in a dog who is otherwise healthy and not on suppressive medications. When the picture is ambiguous, many specialists recommend treating other conditions first, retesting in a few weeks, or starting a carefully monitored therapeutic trial of levothyroxine with the explicit plan to stop the drug if there is no clear response.
Treatment: Lifelong but Straightforward
Once hypothyroidism is confirmed, the treatment is reliable and inexpensive: oral levothyroxine sodium (L-T4), given for the rest of the dog's life. Veterinary-labeled products such as Thyro-Tabs Canine and Soloxine are preferred over human formulations because dogs metabolize thyroid hormone faster than people and need a higher dose per kilogram than a human patient ever would.
The standard starting dose is approximately 0.02 mg/kg twice daily. Many dogs can eventually be switched to once-daily dosing for convenience, while others — especially larger breeds — control better when the dose is split. A recheck blood test is usually scheduled four to eight weeks after starting therapy, with the sample drawn four to six hours after the morning pill so the vet can measure the peak T4 level. Subsequent monitoring is typically every six to twelve months once the dose is stable.
Most owners notice the dog's energy returning within one to two weeks. Weight loss usually follows over the next few months as metabolism normalizes. Coat regrowth is the slowest change to appreciate — three to six months is typical, and the new coat may temporarily come in a slightly different texture or color. Skin and ear infections that had been chronic and recurrent often resolve and stop coming back.
The main risk of treatment is over-supplementation, which can produce signs of thyrotoxicosis: nervousness, panting, increased thirst and urination, weight loss despite a strong appetite, and, in severe cases, a fast heart rate. These signs are reversible by lowering or briefly stopping the medication and rechecking levels. Rare hypersensitivity reactions can also occur. Following the recheck schedule and reporting any new signs to the vet keeps these complications uncommon.
Long-Term Outlook and Quality of Life
The prognosis for a correctly diagnosed and properly treated hypothyroid dog is excellent. With consistent daily medication and periodic monitoring, most dogs live a full, normal lifespan with no evident impact from the disease. Owners who knew the dog before treatment usually describe the change as dramatic: the same dog, but with the energy and shine they had years earlier.
The most important thing an owner can do is take the diagnosis seriously even when the dog feels better. Skipping doses or stopping the medication "to see if it's still needed" causes the original signs to return, often gradually enough that they are missed for months. If you ever doubt whether the medication is still required, talk to your vet about a structured re-evaluation rather than testing the question on your own.
What You Can Do Right Now
If your middle-aged or older medium-to-large-breed dog has been gaining weight without eating more, slowing down on walks, seeking out heat, or losing coat symmetrically along the flanks, ask your vet specifically about thyroid testing — and ask for a panel that includes free T4 and TSH, not just total T4 alone.
