It rarely announces itself dramatically. There is no vomiting emergency, no obvious injury — just diarrhea that won't quit. Soft, greasy, pale, unusually foul-smelling stool that clears up for a day or two, then comes back. The dog seems otherwise bright; the cat still eats. But the mess keeps returning, and over weeks a puppy that should be gaining weight starts to look a little thin and scruffy. That frustrating, come-and-go pattern is the signature of one of the most common intestinal parasites in pets: Giardia.

Unlike a worm you can see, Giardia is a microscopic single-celled organism, and much of what makes it maddening — the way it lingers, spreads through a household, and reappears after treatment — comes down to a tough little survival capsule called a cyst. Here is what the science says about what Giardia is, why young and shelter animals catch it so readily, how vets pin down the diagnosis, and what actually clears it.

What Giardia Actually Is

Giardia is not a worm or a bacterium but a protozoan — a single-celled parasite that lives in the small intestine. It exists in two forms, and understanding the pair explains almost everything about the disease. Inside the gut, the active feeding form, called a trophozoite, attaches to the lining of the small intestine and interferes with the animal's ability to absorb nutrients, which is what produces the classic greasy, poorly-formed stool. As trophozoites pass down the intestine, many transform into cysts — dormant, hardy capsules that are shed in the feces.

That cyst is the infectious form and the reason Giardia is so hard to shake. As the Merck Veterinary Manual and Cornell's canine health experts describe it, cysts are shed already infective, so they need no maturation time in the environment, and they can survive for weeks to months in cool, moist conditions — especially in water. A pet becomes infected simply by swallowing cysts, whether from contaminated water, from sniffing or licking soiled ground, or from grooming cysts off its own coat.

A Common Parasite, Especially in the Young

Giardia is genuinely widespread. Surveys around the world put the point prevalence in dogs somewhere in the range of 10 to 30 percent, with substantially higher rates in younger animals and in places where animals are housed densely. Studies of dogs visiting parks and of shelter populations have reported infection rates on the higher end of that range, reflecting how easily cysts spread wherever many animals share ground and water.

The risk profile is consistent across the veterinary literature: Giardia turns up most in puppies and kittens, shelter and kennel animals, and pets in crowded multi-animal settings. Two features of the parasite make those environments a perfect storm. First, an infected animal can shed enormous numbers of cysts. Second, the infective dose is tiny — it takes only a small number of swallowed cysts to establish a new infection. Add a shared water bowl, a communal run, or a mother grooming her litter, and the parasite moves easily from one animal to the next.

The Signs to Watch For

The central point about Giardia is that plenty of infected pets show no signs at all. Healthy adult dogs and cats frequently carry and shed the parasite while remaining perfectly well, and the relevance of the infection depends heavily on the animal's age and immune status. When disease does appear, it centers on the gut and reflects the parasite's interference with nutrient absorption:

Because these signs overlap with many other causes of diarrhea — dietary indiscretion, other parasites, food intolerance — Giardia cannot be diagnosed on appearance alone. That persistent, greasy, come-and-go quality is a strong hint, but it is only a hint.

Getting a Real Diagnosis

Confirming Giardia takes laboratory testing, and the parasite is famously good at evading a single check. The tricky part is that cysts are shed intermittently — an infected animal may pass millions on one day and almost none the next — so a single negative test does not rule the parasite out.

The Companion Animal Parasite Council recommends a combined approach for reliability. A fecal flotation, ideally with a centrifugation technique, concentrates and reveals the cysts under the microscope. Alongside it, an antigen test (an enzyme immunoassay, or EIA) detects Giardia proteins in the stool even when cysts are scarce. Using both together, sometimes across more than one stool sample collected over several days, catches far more infections than either method alone. A direct smear of fresh, watery diarrhea can also occasionally reveal the motile trophozoites, though they are not reliably present in formed stool.

What Actually Clears It

There is no single drug licensed as the definitive cure, but the standard, well-supported protocols work well. The two mainstays are fenbendazole and metronidazole. CAPC guidance describes fenbendazole (typically 50 mg/kg once daily for three to five days) as a primary option in dogs, and metronidazole (commonly around 10–25 mg/kg twice daily for five to eight days) as another. In cats, either fenbendazole or metronidazole is used for about five days. In stubborn or relapsing cases, veterinarians often combine the two, and controlled studies in home conditions have found that the fenbendazole-plus-metronidazole combination can improve resolution of both diarrhea and cyst shedding compared with a single drug.

Just as important as the medication is what surrounds it, because reinfection is the usual reason a case seems to "fail." Cysts cling to the coat and blanket the environment, so effective treatment pairs the drug with a decontamination routine:

One point that reassures many owners: veterinary experts generally do not recommend chasing a "test-negative" result in an otherwise healthy pet that has stopped having diarrhea. Because dogs and cats can shed cysts long after they feel fine, the goal of treatment is resolving the clinical disease, not necessarily achieving a permanently parasite-free stool sample.

What About People?

Giardia in humans exists, and understandably owners worry about catching it from a sick pet. The reassuring news from the genetics is that Giardia comes in host-associated groups called assemblages. Dogs and cats mostly carry animal-specialized types (assemblages C, D, and F) that are not the usual human strains, and while some pets do carry the potentially zoonotic assemblages A and B, direct transmission from pets to people appears to be uncommon in practice. Most human giardiasis is acquired from contaminated water and human-to-human spread rather than from the family dog. Even so, basic hygiene is sensible: wash your hands after handling feces or cleaning up, and keep young children and immunocompromised people away from the mess during an active infection.

The Short Version

Giardia is common, often silent, and — with correct diagnosis and follow-through — very treatable. The tell is persistent, greasy, foul, come-and-go diarrhea, especially in a young or newly-adopted animal; the confirmation is a combined fecal flotation and antigen test, sometimes repeated; and the fix is a short course of fenbendazole or metronidazole paired with a bath and a thorough environmental clean-up to stop reinfection. Where it goes wrong is treating the drug alone and skipping the decontamination, or panicking over a lingering positive test in a pet that is otherwise perfectly well. Diagnose it properly, treat the pet and the environment together, and that maddening on-again-off-again diarrhea will finally settle.
