FIV has a public relations problem. Because it is often described as "feline AIDS," a positive test still leads some owners to assume the worst and some shelters to treat a cat as unadoptable. The science says something quite different. Most FIV-positive cats look and act completely normal for years. Many live an ordinary feline lifespan. And the research on how the virus actually moves between cats has quietly overturned much of the advice owners were given twenty years ago. Here is what a positive FIV test really means.
What FIV Actually Is
Feline immunodeficiency virus is a lentivirus — a slow-acting retrovirus in the same broad family as HIV. Like all retroviruses, it inserts its genetic material into the cat's own cells, which is why infection is lifelong. FIV targets the immune system, gradually depleting the T-lymphocytes that coordinate the body's defenses. Over years, that erosion can leave a cat unable to fight off bacteria, viruses, fungi, and parasites it would otherwise ignore.
The comparison to HIV is biologically real but socially misleading in one crucial way: FIV is a cat-only virus. It cannot infect humans, dogs, or any other non-feline species. You cannot catch FIV from your cat, no matter how much the two of you share a pillow.
FIV is also less common than its reputation suggests. The Cornell Feline Health Center puts infection rates at roughly 2.5 to 5 percent of healthy cats in North America, rising to 15 percent or higher among cats that are already sick or belong to high-risk groups — unneutered outdoor males, in particular.
How It Spreads (and How It Mostly Doesn't)
The primary route of FIV transmission is deep bite wounds from an infected cat. The virus is present in saliva, but it needs to be injected below the skin to establish infection reliably. That single fact explains almost everything about the epidemiology of FIV: the highest-risk cat in the world is an unneutered male who roams outdoors and fights over territory and mates.
What does not efficiently spread FIV is casual contact. Sharing food and water bowls, using the same litter box, sleeping in a pile, and even mutual grooming are inefficient routes of transmission. This is a genuine departure from FeLV, which spreads readily through exactly those friendly behaviors.
The evidence here is better than most owners realize. A 2014 study by Litster, published in The Veterinary Journal, followed cats cohabiting in two rescue shelters where FIV-positive and FIV-negative cats lived together for months to years, sharing bowls, litter boxes, and bedding, with occasional mild squabbling. Repeat testing found no evidence of transmission to the negative cats. Kittens born to FIV-positive queens in the study also tested negative.
Mother-to-kitten transmission is possible but uncommon in naturally infected queens. There is an important wrinkle, though: a nursing kitten absorbs its mother's antibodies, and because standard FIV tests look for antibodies rather than the virus itself, kittens from a positive mother can test positive without being infected. Those antibodies can persist for up to six months.
The Three Phases of Infection
FIV does not progress in a straight line, and understanding its phases explains why so many positive cats seem perfectly healthy.
- Acute phase (roughly 1–3 months after infection). Some cats develop a fever, mild lethargy, and enlarged lymph nodes. The signs are so mild and so generic that most owners never notice them, and many cats show nothing at all.
- Asymptomatic phase (months to many years). The virus replicates slowly inside immune cells while the cat appears entirely normal. This phase can last the rest of a cat's natural life. Most FIV-positive cats living in homes today are in this stage.
- Progressive phase. In cats whose immune systems eventually falter, opportunistic infections take hold. Notably, not every infected cat reaches this stage.
Symptoms to Watch For
Because FIV itself rarely causes disease directly, the signs owners see are usually the downstream consequences of a weakened immune system. Watch for:
- Chronic gingivitis and stomatitis. Severe, painful inflammation of the gums and mouth is one of the most characteristic findings in FIV-positive cats. A cat with stubborn oral disease is a cat that should be tested.
- Recurrent infections that keep returning after treatment — upper respiratory, skin, eyes, ears, bladder.
- Slow, unexplained weight loss and a dull, poorly kept coat.
- Enlarged lymph nodes and low-grade recurring fever.
- Chronic rhinitis — persistent sneezing and nasal discharge.
- Chronic diarrhea or ongoing digestive problems.
- Behavioral or neurological changes in a minority of cats, including altered sleep patterns and disorientation.
None of these are specific to FIV. That is precisely why testing, rather than guessing from symptoms, is the only reliable way to know a cat's status.
Testing: What a Positive Result Does and Doesn't Mean
The standard in-clinic screening test is a combination point-of-care test that detects FeLV antigen and FIV antibody from a few drops of blood. The 2020 AAFP Feline Retrovirus Testing and Management Guidelines recommend knowing the retrovirus status of every cat, and testing at acquisition, before FeLV vaccination, after possible exposure, or whenever a cat becomes unexplainedly ill.
Three caveats matter enormously:
- The test detects antibodies, not the virus. Antibodies mean exposure. In an adult cat, that almost always means infection — but in a kitten under six months, it may simply mean maternal antibodies. The AAFP advises retesting antibody-positive kittens every 60 days up to six months of age; if the kitten becomes seronegative, it most likely was never infected.
- Vaccination causes positives. A cat vaccinated against FIV will test antibody-positive indefinitely, indistinguishable from a naturally infected cat on a standard test. This diagnostic interference, alongside questions about efficacy against the range of FIV subtypes, is a major reason the FIV vaccine is no longer commercially available in the United States and Canada. It remains available in some other countries, including Australia and New Zealand.
- Confirm before you act. A positive screening result should be confirmed with a second, different method — typically Western blot or PCR. Given how consequential a diagnosis can be for a cat's future, no cat should be rehomed, isolated, or euthanized on the basis of a single in-clinic test.
Life Expectancy: The Number That Surprises People
Here is the finding that most contradicts FIV's reputation. Cornell reports that cats with FIV commonly live average life spans, provided they are not also infected with feline leukemia virus. Roughly 18 percent die within five years of infection and another 18 percent are alive but showing illness from immune suppression; the remainder appear normal over that period and many go on to live long lives with only occasional bouts of illness.
Co-infection is the real danger. An FIV-positive cat that also has FeLV faces a far worse outlook than either virus alone, which is one more reason the combination test matters.
Caring for an FIV-Positive Cat
There is no cure for FIV, and treatment is supportive rather than antiviral in most cases. The management plan is refreshingly practical:
- Keep the cat indoors. This protects other cats from exposure and, just as importantly, protects an immunocompromised cat from the pathogens, parasites, and fights waiting outside.
- Neuter. Removing the hormonal drive to fight sharply reduces the behavior that transmits the virus in the first place.
- Veterinary visits at least twice a year, with a physical exam, weight check, bloodwork, and urinalysis. Early detection of secondary problems is the single biggest lever on outcome.
- Aggressive dental care. Given how common severe oral disease is, dental health deserves real attention rather than an afterthought.
- Feed a complete, balanced, cooked diet. Raw food carries a bacterial and parasitic risk that an immunocompromised cat is poorly equipped to handle.
- Treat illness promptly and thoroughly. Infections that a healthy cat would clear on its own may need earlier and longer intervention.
- Stay current on parasite prevention and discuss the vaccination plan with your veterinarian.
Can FIV-Positive and Negative Cats Live Together?
Twenty years ago the answer was a flat no. Today, based on the Litster shelter data and similar findings, many veterinarians and shelters take a more nuanced position: the risk of transmission within a household of compatible, neutered adult cats that do not fight appears to be very low. Many shelters have accordingly dropped blanket restrictions on adopting FIV-positive cats into homes with negative cats.
That is a risk assessment, not a guarantee, and it depends on the specific cats. A household where cats genuinely get along is a very different proposition from one with ongoing territorial aggression and bite wounds. Discuss it honestly with your veterinarian, introduce new cats slowly, and be prepared to separate if fighting develops.
The bigger point is that an FIV diagnosis is not the sentence it is often taken to be. A positive cat kept indoors, neutered, well fed, and seen regularly by a veterinarian has a real shot at a full and comfortable life — which makes FIV-positive cats some of the most unfairly overlooked animals in any shelter.
