It usually starts with the scratching. A cat digs at one ear with a hind foot, shakes its head as if trying to fling something loose, then goes back to digging. Look inside and you find it: a dry, crumbly, dark-brown debris that looks unnervingly like coffee grounds, packed down into the ear canal. That gunk is the calling card of one of the most common — and most contagious — parasites in cats: the ear mite.

Ear mites are a nuisance rather than a catastrophe for most cats, but they are miserable for the animal, they spread through a household with remarkable ease, and left alone they can set off secondary infections that do lasting damage. Here is what the science says about what these mites are, why kittens and outdoor cats catch them so readily, and how modern one-dose treatments finally clear them.

What an Ear Mite Actually Is

The feline ear mite is Otodectes cynotis — a tiny, eight-legged, spider-like arachnid barely visible to the naked eye as a moving white speck. Unlike mange mites, it does not burrow. As the Companion Animal Parasite Council and Cornell's Feline Health Center describe it, Otodectes is a surface parasite that lives on the skin lining the ear canal, feeding on ear wax, skin oils, and tissue debris. Its whole world is that warm, dark, waxy tunnel.

It is also, by a wide margin, the leading cause of feline ear disease. Veterinary parasitology references attribute somewhere between 50% and 85% of all cases of otitis externa (inflammation of the outer ear canal) in cats to ear mites — a far higher share than in dogs, where the same mite accounts for only about half as many cases. When a cat has an angry, itchy, gunk-filled ear, mites are the first thing a vet thinks of.

A Three-Week Life Cycle That Explains the Re-Infestations

Understanding the mite's life cycle is the key to understanding why casual treatment so often fails. The entire cycle — from egg to reproducing adult — takes about three weeks and plays out entirely within the ear canal, passing through four stages: egg, six-legged larva, two eight-legged nymph stages, and the adult. A female lays her eggs on the canal wall; roughly four days later a larva hatches, feeds, and molts twice before maturing. Adult mites then live for around eight weeks, breeding continuously the whole time.

Here is the practical problem: most topical treatments kill the mobile mites — larvae, nymphs, and adults — but do little to the eggs. An egg laid the day before treatment can hatch a week later into a brand-new mite, restarting the infestation. This is exactly why older ear-drop protocols required repeated dosing over several weeks, and why the modern products that persist in the body long enough to catch newly hatched mites have been such an improvement.

Who Gets Them, and How They Spread

Ear mites spread by direct contact. They do not fly or jump; they simply walk from one animal to another during the ordinary close contact of a shared bed, a grooming session, or a mother nursing her litter. That transmission route explains the classic risk profile documented in the veterinary literature: mites turn up most often in kittens, feral and free-roaming cats, shelter populations, and multi-pet households. A queen with mites will almost invariably pass them to every kitten she raises.

The parasite is not host-specific, either. Otodectes cynotis moves readily between cats, dogs, ferrets, and rabbits, which is why a single infested newcomer can seed an entire mixed household. If one animal in a home is diagnosed, every in-contact pet needs to be treated — even the ones showing no signs — because a light, symptom-free infestation in one animal will simply re-seed the others once the treated cat is clear.

The Signs to Watch For

The hallmark of an ear-mite infestation is intense, persistent irritation. The mites' movement and feeding provoke an inflammatory and often allergic reaction in the canal, which is why the itch is frequently out of proportion to the number of mites present. Typical signs include:

The most important secondary complication is the aural hematoma: violent head shaking can rupture a blood vessel inside the ear flap, causing it to balloon with blood into a firm, painful swelling that often needs surgical drainage. Untreated mites also open the door to secondary bacterial and yeast infections, and chronic, severe infestations can damage the canal and eardrum enough to affect hearing. None of this is inevitable — but it is why "just a few mites" is worth taking seriously.

Getting a Real Diagnosis

It is tempting to diagnose ear mites from the coffee-ground gunk alone, but that is a mistake, because bacterial and yeast ear infections can look almost identical and are treated completely differently. A proper diagnosis is quick and cheap. A veterinarian will typically examine the canal with an otoscope — under magnification the tiny white mites can sometimes be seen moving against the dark debris — and confirm it by smearing a sample of the discharge onto a slide with mineral oil and looking at it under a microscope, where the mites and their eggs are unmistakable. Because the treatments for mites, bacteria, and yeast differ, that five-minute check saves money and prevents weeks of using the wrong product.

What Actually Clears Them

Ear-mite treatment has improved dramatically. The mainstays today are two drug families: the macrocyclic lactones (selamectin, moxidectin, and ivermectin) and the newer isoxazolines (such as lotilaner and fluralaner). Veterinary guidance now generally favors systemic products — a spot-on applied to the skin or an oral dose — over old-fashioned ear drops, precisely because they persist in the body long enough to keep killing mites as new ones hatch across the three-week cycle.

The evidence is strong. In naturally infected cats, a single topical dose of selamectin (at the labeled 6 mg/kg) eliminated mites in roughly 94–100% of cats by day 30 in controlled studies, and newer isoxazoline products have shown similarly high cure rates from a single treatment. If a veterinarian does prescribe a non-residual ear medication instead, the classic advice still holds: plan on re-treating about three weeks later to catch the generation that hatched from surviving eggs. Alongside the miticide, gentle cleaning of the debris out of the canal — done or guided by your vet — helps the medication reach the skin and relieves the cat's discomfort faster.

Two cautions are worth repeating. First, never reach for a leftover ear product or an over-the-counter remedy without a diagnosis; if the real problem is a bacterial infection, a mite drop will not help and delays proper care. Second, treat every pet in the home at the same time, and give the cat's bedding a hot wash, since mites can survive briefly off the host in the immediate environment.

Ear mites are common, intensely itchy, highly contagious, and — with a correct diagnosis — genuinely easy to cure. The coffee-ground gunk and the relentless scratching are the tells; a quick microscope check confirms it; and a single dose of a modern systemic miticide, applied to every animal in the household, usually ends the story. Where it goes wrong is guessing instead of diagnosing, treating one pet and not the rest, or ignoring the itch until a burst blood vessel or a secondary infection turns a minor parasite into a real problem.
