Before modern vaccines, canine distemper was the disease dog owners feared most — a virus that could sweep through a town and empty its kennels. Vaccination has made it rare among well-cared-for pets, but the virus never went away. It still circulates constantly in raccoons, foxes, skunks, and coyotes, still causes devastating outbreaks in shelters, and still kills unvaccinated dogs every year. Unlike many infections, distemper attacks three body systems at once — respiratory, digestive, and nervous — and there is no drug that kills the virus. Prevention is everything.
Here is how distemper spreads, how to recognize it at each stage, what treatment can and cannot do, and why the vaccine your veterinarian recommends is considered non-negotiable.
What Canine Distemper Is
Canine distemper virus (CDV) is a morbillivirus in the paramyxovirus family — a close relative of the human measles virus. After a dog inhales the virus, it first multiplies in the lymphatic tissue of the respiratory tract, then spreads through the bloodstream to the surfaces of the airways, gut, urinary tract, skin, and eyes, and finally to the brain and spinal cord.
Along the way, CDV does something especially dangerous: it suppresses the immune system. Infected dogs become vulnerable to secondary bacterial infections — pneumonia in particular — which are responsible for much of the disease's damage. Despite the similar name, canine distemper has nothing to do with feline distemper (panleukopenia), which is caused by an unrelated parvovirus.
How Dogs Catch It
Distemper spreads mainly through airborne droplets from coughing and sneezing, much like a human cold. Direct contact with an infected animal, or with freshly contaminated bowls, bedding, and equipment, also transmits the virus, and infected mothers can pass it to puppies through the placenta.
Two features make the virus hard to stamp out:
- Wildlife reservoirs. Raccoons, foxes, skunks, coyotes, wolves, and ferrets all carry and die from CDV. A backyard raccoon or an urban fox can seed infection into a neighborhood with no sick dog anywhere in sight.
- Prolonged shedding. Dogs that survive infection can continue shedding virus for several months, quietly exposing other animals long after they seem recovered.
The one piece of good news: because CDV is an enveloped virus, it is fragile in the environment. Routine disinfectants, heat, and sunlight destroy it, and it typically survives only hours at room temperature — a sharp contrast to parvovirus, which persists in soil for a year or more.
The Warning Signs, Stage by Stage
Signs usually appear one to six weeks after exposure, and the disease tends to unfold in phases:
- First phase — fever and discharge. A transient fever appears within about 3–6 days of infection and may pass unnoticed. A second fever follows, accompanied by the classic early picture: watery to pus-like discharge from the eyes and nose, lethargy, and loss of appetite.
- Respiratory and digestive signs. Coughing, labored breathing, vomiting, and diarrhea develop as the virus spreads along the airway and gut linings. Secondary bacterial pneumonia is common.
- Skin and pad changes. Some dogs develop thickened, hardened skin on the nose and footpads — the hyperkeratosis that gave distemper its old name, "hardpad disease."
- Neurologic phase. Days to weeks later — sometimes after apparent recovery — the virus reaches the nervous system. Signs include head tilt, circling, muscle twitches (myoclonus), rhythmic "chewing-gum" jaw movements, partial or full paralysis, and seizures.
Not every dog follows the full sequence; some show mainly respiratory disease, others progress rapidly to seizures. Any unvaccinated dog with eye and nose discharge plus fever, coughing, or GI signs needs veterinary attention immediately — and the clinic should be warned in advance so the dog can be isolated from other patients.
Diagnosis and Treatment
Veterinarians diagnose distemper from the clinical picture, vaccination history, and laboratory confirmation — most often PCR testing on swabs from the eyes, nose, or throat, or on blood and urine. Because the early signs mimic kennel cough and other respiratory infections, testing matters: a distemper diagnosis changes isolation, prognosis, and monitoring entirely.
There is no antiviral drug approved to treat CDV. Treatment is supportive care, ideally in an isolation ward: intravenous fluids for dehydration, antibiotics for secondary bacterial pneumonia, anti-nausea medication, nutritional support, and anticonvulsants if seizures develop. Survival depends heavily on the strain of virus, the dog's age and vaccination status, and the speed of care. Mortality is commonly cited at roughly 50% in adult dogs and up to 80% in puppies, and the outlook worsens sharply once severe neurologic signs appear.
Dogs that survive may carry lifelong souvenirs: permanent muscle tics, enamel defects on teeth that were developing during the infection, and hardened footpads. Many survivors, however, go on to live full, happy lives — some with a harmless twitch as the only reminder.
Prevention: One of Veterinary Medicine's Great Success Stories
Distemper is a core vaccine for every dog, as designated by the American Animal Hospital Association (AAHA) and the World Small Animal Veterinary Association (WSAVA). It is the "D" in the DAPP/DA2PP combination shot, and the modified-live vaccine is highly effective. The standard protocol is:
- First dose at 6–8 weeks of age.
- Boosters every 3–4 weeks until at least 16 weeks of age — maternal antibodies can block earlier doses, which is why finishing the full series matters.
- A booster one year later, then revaccination every three years.
Until a puppy has completed the series, treat it like the immunologically vulnerable animal it is: avoid dog parks, unfamiliar dogs, and places where wildlife concentrates. Feed pets indoors rather than leaving food outside that attracts raccoons and foxes, secure trash cans, and keep water bowls out of the yard overnight. Ferret owners should know their pets need their own distemper vaccination — CDV is almost invariably fatal in unvaccinated ferrets.
Adult dogs whose vaccines have lapsed are not automatically protected by shots they received as puppies. If you have adopted a dog with an unknown history, a simple booster — or a titer test, if you prefer — closes the gap.
The Bottom Line
Canine distemper is a disease with a cruel arc: cold-like signs that look manageable, followed weeks later by seizures that often are not. There is no cure, treatment is a fight for survival, and the virus is always nearby in wildlife — yet a complete vaccine series and routine boosters give near-complete protection. Keep puppies away from high-risk places until their series is finished at 16 weeks, keep adult boosters current, and call your veterinarian at the first sign of eye and nose discharge with fever in any unvaccinated dog. With distemper, the vaccine appointment you keep is the emergency you never have.
